Registration Information
Thank you for choosing Our Lady of Charity Private School as the primary care giver and educator of your child (ren). It is our pleasure to have you join our family. The following information is going to be vital in your child's preparation for the Education at our school. Please read it carefully and if you have any questions please do not hesitate to contact us.
Supply Lists:
All of the following material is required from all students and must be brought
in on the first day of school.
Nursery:
Box of tissue, box of baby wipes, folder for work sent home, 2 glue sticks, 1 pack of construction paper, 1 picture of child's entire family, small blanket for nap time ( NO larger than 30" x 40") travel size pillow ( no larger than 6" x 10") small book bag WITH OUT wheels! (NO larger than 10" x 12")
Pre-Kinder:
Box of tissue, box of baby wipes, large coloring book, 1 folder for work sent home, 2 glue sticks, 1 pack of construction paper, small blanket for nap time (NO larger than 30" x 40"), small book bag WITH OUT wheels! (NO larger than 10" x 12"), 5 tennis balls
Kindergarten:
Box of tissue, box of baby wipes, large coloring book, folder for work sent home, small book bag WITH OUT wheels! (NO larger than 10" x 12") 1 pack of construction paper, 2 glue sticks, 1 handwriting tablet, 2 fat beginners pencils, I pencil box, 5 tennis balls
First thru Fourth: 1 handwriting tablet, 12 composition note books (sewn pages), 4 folder with pockets, 1 pack construction paper, plenty of pencils, 2 glue sticks, 1 ruler, 1 safety scissor, 1 sharpener with cover, 1 box of crayons, 1 box of washable markers, 1 English Elementary Dictionary, 10 stretch book covers, and book bag WITH OUT wheels, 5 tennis balls
Fifth thru Eighth: 12 composition note books (sewn pages), 4 folder with pockets, 1 pack construction paper, pencils, pens, 1 red pen, 2 glue sticks, 1 ruler, 1 scissor, 1 protractor, 1 compass, 1 sharpener with cover, 1 box of color pencils, 1 box of washable markers, 1 English Dictionary, 10 stretch book covers, book bag WITH OUT wheels, 5 tennis balls.
All of the above mentioned items must be labeled with child's full name.
Uniforms:
Nursery: P.E. Shirts and P.E. shorts. Blue or grey pants during winter, sneakers
Pre kinder:
Girls: Navy blue jumper and baby blue blouse with Peter Pan collar,
Kinder: white socks and black shoes.
Boys: Navy blue pants with baby blue polo shirt. Black belt, white socks and black shoes (not sneakers)
First thru:
Girls: Navy blue jumper knee length and baby blue blouse with Peter Fifth Pan collar white socks and black shoes.
Boys: Navy blue pants with baby blue polo shirt, black belt, white socks and black non marking shoes (not sneakers)
Under the school uniform students must wear the P.E. uniform.
Sixth thru:
Girls: Navy blue skirt knee length and baby blue blouse with Peter Pan
Eighth: collar, white sock and black shoes.
Boys: Navy blue pants with baby blue polo shirt, black belt, white socks and black non marking shoes (not sneakers)
Both are to wear the P.E. uniform underneath the school uniform,
Both are to wear a mandatory navy blue vest on top of the shirt or blouse.
Attention all Students!
Physical Education Uniforms and Sweaters can be purchased at the school.
* All school uniforms must have the school logo and Initials on them. If they do not have them or if any part of the school uniform is missing students will be sent home.
Baggy uniforms or uniforms that are too large or over sized will not be allowed.
* Only Navy blue school sweaters are allowed and must have school logo on it.
*Girls may wear tight leggings underneath their skirt in the winter.
*No excessive jewelry or electronic devices are allowed.
Girls: Hair must be one color and well groomed and kept out of the face.
Nails must be short and unpainted no acrylics or 'fake nails'.
Boys: Hair must be kept short (not to touch the shirt collar), well groomed and one color.

Registration Information

PAYMENT METHODS:
Our school offers several different payment plans to facilitate the payment process.
Special arrangements and scholarships will be handled directly through the main office. Contact us with any questions.
We also offer the following payment options:
Ten Payments Plan- 10 payments by the 5th day of each month.
Two Payments Plan- 2 payments on August and January (discount allowed).
Annually- 1 payment on August15th (discount allowed).

No after school care for students from 6th to 8th grade. |

Our school offers quality after care for students attending local public schools. Contact main office for more information.

NOTES:
- Tuition is due within the first five days of each month.
- If payment is 15 days past-due, we must ask you to withdraw your child.
- All late payments are subject to a late fee.
- Charges will apply to any check returned from bank.
- Audits to account are subject to service charges.
- Book fees are due with the first month's payment.
- Books are school property.
- Charges will apply in case of damage or loss of books.
- Arrangement for Supervised Homework should be made in the Office.
Important Health Information for the Parents.
In order for students to achieve their very best they need to BE a their very best. This includes a Healthy Breakfast, Plenty of Sleep, Arriving to school school on time and well prepared and most of all feeling healthy. Students that are sick should not attend school. The following are guide lines, given by the Department of Health and Social Services, to help you decide if you child is well enough to attend school.
You are the best judge of your child’s health and we trust you will not bring a sick child to school. However, if it is opinion of the teaching staff that your child is sick, we will call you to pick up your child. The following will be considered in determining if your child must stay home or be picked up to go home:
- Fever of 100` F or 38`C or more
- Inflammation of the eyes
- Vomiting
- More than one incidence of diarrhea
- Communicable disease
- Unknown rash
If your child is sent home due to illness s/he cannot return to school until s/he has been free of symptoms for 24 hours. This is to allow your child ample time to recover and stop the spread of illness to other children.
- Please keep your child at home is they have any of the following signs
- Fever
- Pain
- Frequent Coughing
- Sighs of pink eye
- Vomiting
- Diarrhea
- Excessive nasal discharge
Contagious diseases must be reported to the school by the parents as soon as it is diagnosed, so that other parents may be notified of the exposure.
Important information:
- Medical emergency information must be kept current in your child’s file.
- All immunizations must be kept current.
- Please let us know if your child has any allergies or food restrictions.
Medication:
The school will NOT administer any type of medication not prescribed by a doctor. Parent may bring prescribed medication in it’s original bottle, with child’s name on it to the school office. Once a ‘Request to give medication’ form is filled out and signed, we will follow the directions stated on the bottle.
Please do not put medicine in book bags or lunch boxes.
School First Aide
Minor injuries will be treated with hydrogen peroxide, water, soap, ice and Band-Aids. In the event of a serious accident, parents or other persons listed on emergency card will be contacted. If no one can be contacted or located we will call the Physician listed on emergency card, permenant file and/or 911.
An incident report will be filled out and given to the parent at the front office.
